The Law Practice of Dennison Keller, LLC, a small boutique elder law firm, seeks to fill an opening for a full-time Elder Care Coordinator (ECC).

Position Overview:  The ECC is a client advocate within the firm and an advocate for the client’s care rights in the community.  The ECC serves as a liaison between the elder and his or her family and the appropriate community resources available in Southwest Ohio, and Northern Kentucky. 

Responsibilities:

  1. Client Relations
    1. Meet with the client in his or her current environment to evaluate his or her capacity and assess gaps between current and required care.
    2. Help the client and/or his or her family arrange services for the elder based on the ECC’s knowledge so that the client’s care and quality of life are maximized.
    3. Counsel the family on care issues and concerns.
    4. Help the client find good care, whether inside or outside the home.
    5. Serve as an ongoing resource to client and family by answering questions, visiting the client and periodically reviewing the care plan in view of changed circumstances.
    6. Advocate the position of the elder within the firm to assure best practices and continuous improvement in the delivery of our services.
    7. If the client is in a medical facility, review the chart, including medications.
    8. Draft assessment reports to give clients an independent third-party view of the care they are receiving and offer suggestions on how that care may be improved. 
    9. Counsel clients on questions they should be asking their care team.
    10. Provide emotional support to clients and their families as the client moves down the long-term care continuum.
  1. Community Relations and Marketing
    1. Stay current on local care options in the community, including the variety of options and their quality.
    2. Represent the firm to the healthcare and long-term-care community through frequent visits. Create positive relationships with management staff for purposes of client care maximization and referrals.
    3. Participate in professional associations, both to enhance skills and to communicate the firm’s services.
    4. Educate the community about the firm’s services and service model through speaking engagements, articles and networking.
  1. Management Responsibilities
    1. Maintain records on “TimeMatters,” or similar software, to allow firm-wide access to real-time client information.
    2. Maintain confidentiality of all client information, including the fact that someone is a client, except as necessary to obtain services for that client, or with the client’s express, written approval.

Qualifications:

  1. R.N. and/or Bachelor’s level degree in Nursing with Nursing experience preferred. Dual licensure in Ohio and Kentucky a bonus. Emphasis in geriatrics preferred.
  2. Will consider Bachelor’s level degree in Social Work.
  3. Will consider a background in Geriatric Care Managing.
  4. Broad and commanding knowledge of the services available to local senior citizens
  5. Excellent communication skills, in both individual settings with clients and families, and in the community.
  6. Knowledge of Medicare coverage and appeals preferred.
  7. Excellent organizational and time management skills.
  8. Computer skills, including proficiency in Microsoft Word and Outlook are a must. Willingness and ability to learn new computer software programs are a must.
  9. Ability to work independently and with minimal supervision and direction.
  10. Willingness to contribute to and work within our team environment.
  11. A passion for working with the elderly.
